How to format a good essay structure?

I am supposed to have my essay completed by tomorrow but I procrastinated until today to start it. I don't know how to format my essay in order for it to look good and actually make sense. I know you need an intro and conclusion paragraph but the middle ones are what confuse me. My teacher always says they are no cohesive and don't have a set theme for each. What does this mean?
